If the characters were never together to begin with and one is just watching it is not the same thing imo. Many ntr doujins have great art. As a outsider looking in for most doujins it is easier to self insert as the random person than some odd harem king mc. In other cases some doujins can be funny like with some Monogatari ntr doujins.
