Maybe Elfen Lied?

I should  check my list as I don't remember well.
Higurashi has some really gruesome scenes yes, but it isn't as consistantly gory as Elfen Lied.
Maybe another one I should mention is Berserk.
Also remembered I watched Dokuro-chan. That one is also really gory.

Hard to say, there's different types of violence and different types of graphic depictions.

For realistic depictions of violence, probably graveyard of the fireflies or in this corner of the world.

For sexual violence, probably Kara no Kyoukai movie 3.

For in general violence and blood splatter, Psychopass or The Idaten Deities.

Hellsing ultimate easily, blood splashy splash every episode. Claymore (manga) have some of the best gory violence as well.

I'm surprised Attack On Titan isn't mentioned yet lol.
Human bodies got chomped in half and heads eaten feels very violent to me. The manga feels a lot more violent than the anime because is the rougher drawing /strokes, compared to the clean anime tho.

Urotsukidouji is definitely known for its ultraviolence (and.....other stuff), though i have yet to watch more than clips of it so far. i'm just aware of its legacy

but if we're talking things i've seen all the way through recently, Iczer-1 had impressive gorey body horror violence.

Violent or disturbing?
I find they have different approach, an extremely violent anime might not be disturbing to watch (such as Hell Paradise/ Chainsaw man)
However a moderately violent anime can be very disturbing to watch, such as a certain episode of Made in Abyss.

It depends on what you mean by violent. A lot of anime include people beating the shit out of each other, which is by definition extremely violent, but I assume that isn't what you mean. I assume you're looking for brutality and cruelty.

Higurashi is known for it's brutal murders, which I can definitely confirm, but there aren't actually that many of them. Has the most brutal murders, yes, but the most brutal anime? Considering how sparingly they use their gore, I can't say it applies to the whole show.

Another is the one that I would say applies to the whole show, although I think their overuse of cruel and unusual deaths is just kinda funny.

It's definitely some of the hyper violent 80s-90s OVAs I watched, but I'd say Devilman just because it's the one that's still in my mind.

That depends on what you mean by violent. Do you mean the most gorey? The one with the most fighting? The anime with the biggest emotional reaction to an act of violence? Like, what are we using to gauge the violence?

Genocyber comes to mind with children getting shot by a minigun on screen with lots of detail at that.

Cant recommend it though, as it becomes a nothing-ever-happens show later on.

So far nothing tops Genocyber in terms of the sheer amount of graphic, detailed gore, even something like Corpse Party. New Fist of The North Star also comes to mind for this. In terms of brutal, fucked up violence, Violence Jack: Evil Town probably takes the cake. Other ones that come to mind but are more on the WTF side of things are Ninja Resurrection and Apocalypse Zero, both are turbo shit but worth a watch because of how batshit and repulsive the content in them are.
In terms of gore to runtime ratio, Pupa wins. Each episode is 4 minutes and there's cringe-inducing (as in, makes you squeamish but also yeah the show is also very cringe as it butchers the source material) gore in almost every scene.
In terms of sheer amount of violence, Hellsing Ultimate, Blood-C and Fist of The North Star: The Movie (1986) I think are top contenders. Full of gory action.

For hentai, off the top of my head, Urotsukidoji, Gedou Gakuen, Euphoria and Bondage Game. There's definitely more gory ones I haven't watched or heard of though, so I'll be on the lookout for those.

For manga, the aforementioned manga also make the list, though be sure to check out Gantz, Baki and Ichi The Killer as well, they're fantastic.

For visual novels, there's Saya no Uta, Gore Screaming Show and Euphoria, though I've yet to check them out.

HUGE warning for the following section. Many of these works are some of the most fucked up things you'll ever find in this entire medium, period. Read them at your own risk, even as a guro enthusiast, I mean it when I say that these are not for the faint of heart.

For doujinshi, check out Mai-chan no Nichijou (or anything by Uziga Waita), Tasatsu Shigan (or anything by Juan Gotoh), Kago Shintarou's works and Amagappa Shojogun's works. Also check out Erogros Anthology and Jigoku no Kisetsu.

Seen a lot. Here are two random examples:
Shoujo Tsubaki: a brutal murder of a puppy. Eye licking. Cripples and mutants having sex, torturing and killing people/each other.
Mad Bull 34. In a funny way. 
Violence Jack (Devilman's sequel btw) is much more violent than Devilman itself.
One of the most violent things I've seen in my life is Mai-chan no Nichijou manga. That notorious episode:
