I believe that it is wrong to translate the title as "The Elf, The Boy And". From what I understand, the second と doesn't have the same meaning as the first. I think it doesn't say "X and Y and Z" but rather it is a slightly different (perhaps archaic?) way to say "X and Y". The second と doesn't at all mean that another object is expected after it. Therefore, we should translate the title as just "The Elf and the Boy". I'm not a language expert so I asked about this on StackExchange and so far the only response seems to confirm my understanding: https://japanese.stackexchange.com/questions/30969/x-%e3%81%a8-y-%e3%81%a8-how-does-the-second-%e3%81%a8-affect-the-sentence The person apparently has been studying Japanese for over 15 years, of course it doesn't automatically make their answer true but it's just something to consider.
