I don't think it's anything complicated :p some stories focus on one couple and others focus on multiple ones. Some stories have more straight characters and/or couples and some less (or none). Some stories are more on a serious side and romance is often secondary, while others are just outright silly and full of smut. Just like with any other media. But I admit that such a thought crossed my mind a couple of times. I was like "it feels so strange that all of them
just happen to be gay". Not that I mind it, or that it's impossible (multiple people already pointed out why it's entirely possible and even realistic), but I guess in certain stories I wanted a more realistic
justification in particular :) because not all BL is a slice of life. If it's some grand fantasy story or sci-fi with big worlds, you can't help but wonder "Why is every guy I meet gay? Why am I so lucky?" :p Although, I guess it mostly concerns love interests in visual novels, so it also makes sense. I don't know, I guess it all comes down to the suspension of disbelief, and you just roll with it. Overall, I don't really care and fine with whatever, as long as I'm able to like it.

There is definitely profound and deep storytelling to be found, but it's mostly in visual novels and manga :) not that they lack gay action either lol but still :p and sometimes you're 20+ hours in and suddenly the first sex scene happens, and you're like "oh...right, I'm playing a BL game" lol

I believe the assumption on OP's part is that BL fans enjoy both straight and gay relationships. It's not an unthinkable idea, given that the majority enjoy straight relationships.
I'm pretty sure Sakura and Shaoran are still my favorite couple, so I guess that assumption works in practice even for people who aren't directly involved in straight relationships :p

It most likely has something to do with the need for target audience (straight females) to have the "official" ability to self-insert if they want :) I don't feel like it happens "usually", at least from my experience, but it's not like I always pay attention to someone declaring they're gay lol it probably heavily depends on a number of factors: the story itself (simple slice of life or something grander), the author (male or female, personal preferences), the year of publication (older vs newer) etc.
