A list of disturbing mangas where the queerness of the protagonists holds a central place in the plot, because i am personally growing increasingly frustrated and/or bored with the innocuous queer representation in utterly sterilised mainstream medias.

These queer mangas are ambivalent, some of them might deal in harmful tropes, but they are interested in queerness, and won't treat it as fragile porcelain or unstable explosive.

TW everything you can think of, like this stack is not a most-hardcore-stuff-you-can-read marathon list, but honestly the only reason it's not classified +18 is because i didn't include any Gengoroh Tagame.
Sleeping Dead
Manga, 2 vol, 2020 Me:- Author:-
If Reanimator (1985) was a cooking show
MW
Manga, 3 vol, 1976 Me:- Author:-
Do you wish Urasawa's Monster were gayer ?
Do you have a dark, deeply rooted catholic guilt that gives you the hots for beefy priests ?
Would you like to fetishise a serial killer like seemingly everyone else in this wretched epoch without trivialising the deaths and traumas of real victims ?
Would you be interested in witnessing a never-ending suite of horrifying acts committed by a vicious, fascinating and perverted homosexual right out of the wettest fantasies of a raging homophobe AND a pretty fun thriller AT THE SAME TIME ?

well that's your manga right here, plus the english version's great.

TW: everything, AND this was written in 1976 so it's always treated horrendously bad
MADK
Manga, 3 vol, 2017 Me:- Author:-
this reading should come with a PSA about the dangers of putting your privates anywhere near raw meat

TW: cannibalism, glory and gore, even saying that the consent is dubious here feels like a distasteful joke
Sabishisugite Lesbian Fuuzoku ni Ikimashita Report
Manga, 1 vol, 2016 Me:- Author:-
nothing hits more that non-sacrilised-queer-people spot than reading a brutally honest report on the real fucking deal.

TW: eating disorder, risks of relating to things you didn't want to think about ever
Pornographer
Manga, 1 vol, 2015 Me:- Author:-
Maybe the more PG title of the stack as no one here gets quite killed and/or sexually assaulted. Too bad the title makes it impossible for you to read it in public tho sksksk

Also one of the more accessible in my mind as we follow a depressed loser with no ethics that fakes being straight for his job, and as BL enjoyers with a MAL account i think we can all relate to that.

Jokes aside, Maki Marikudo gives and dangerously accurate depictions of how untreated mental illness and low self-esteem issues can fuck up your life and actually turn you into the piece of shit you see in the mirror.
I love me a problematic bisexual, i love me him finding a labradorish gym bro to love him despite his poor personality, but these 3 tomes will most of all stay with me for a long time both as a cold call-out like a glass of water in the face, and a gentle, judgment-free hug to remind me that life is not over despite of how the past went.
Stigmata: Seikon Sousa
Manga, 1 vol, 2014 Me:- Author:-
for yall with blood and wounds kinks out there you sick dogs <3

TW: some mentions of r*pe and lotsa blood
Stigmata: Aikon
Manga, 2 vol, 2019 Me:- Author:-
same than before but this time the author became open about her BL proclivities and Harry Clarke influences

TW pretty precise and graphic mentions of r*pe, lotsa blood, and GAY HOMOSEXUAL LOVEMAKING
Gesshoku Kitan
Manga, 1 vol, 2015 Me:- Author:-
If you're a fan of Edogawa Ranpo, if you liked Litchi Hikari club, if you needed yourself a good ol' gruesome immoral splatfest or were looking for an actual horror-mystey BL, how lucky you are because Syundei has your back and has it just as good as you'd expect from an author that mastered horror storytelling.
Oni to Tengoku
Manga, 2 vol, 2016 Me:- Author:-
ok let's not be hypocritical because this manga is popular: Tengoku is a pedophile, that's just what it is, i loved this manga as much as you did but if you can't take in this character as a child molester i don't think you're being honest with yourself here.

that being said.

(i hate that i came to end a sentence like that with a "that being said")

that being said, the relationship between him and that repressed middle-aged straight-until-wet musty mop of a man is just. so fucking compelling.
I believe everything around Tengoku doesn't make much sense because the story somehow believes he's a humane character (despite the fact that he begins as a predator AS STATED BEFORE), but this relationship works, it just works so well...
Boku wa Mari no Naka
Manga, 9 vol, 2012 Me:- Author:-
i'm grateful for this author to have given to our fellow cisgender at least a taste of the absurdist horror being transgender can sometimes be.
Boku ga Otto ni Deau made
Manga, 1 vol, 2020 Me:- Author:-
I'm only adding this to say that the OG essay novel is much more brutal and honest, and thus far more interesting, than this sweetened adaptation.
Boy Meets Maria
Manga, 1 vol, 2017 Me:- Author:-
if you got tricked by the pink, cute cover and expected to read something sweet and wholesome i'm so, so sorry for you.
I mean it does get pretty wholesome eventually, but it still goes through [TW: MOST VIOLENT AND BRUTAL DEPICTION OF CSA I'VE EVER READ]

Fabulous manga by a fabulous author, r.i.p Peyo/Eguchi Kōsei, the void you left will never close up.
Smells Like Green Spirit
Manga, 2 vol, 2011 Me:- Author:-
This manga talks about transness, queerness at large, and p*dophilia, talks about it in a rough, brutal and in-your-face way. The portrayal may be awkward and not 100% safe, but istg look at this work of pure sincerity, kindness and desesperation and tell me you want to call it out on twitter dot com.
I love this weird little manga to death and revisit it regularly.

TW: CSA-related stuff not actually happening but it's an important plot point, so it's never graphic but damn is it hard to stomach.
Yuureitou
Manga, 9 vol, 2010 Me:- Author:-
well well well those people sure be gay and sure do crimes

TW: murder gore and everything that you would find in an Edogawa Ranpo novel, one char is weird about teen boys and while crossdressing, genderfluidity and transfemininity are subjects actively discussed while not being demonized at all, there's a JKR-level of perverted murderous trans woman trope towards the end that makes me roll my eyes so hard that i can make scoobi-doos with my optical nerves.

also this is one of my fav manga ever for some reason
Hikaru ga Shinda Natsu
Manga, ? vol, 2021 Me:- Author:-
read this. DO NOT look up anything about it. just read it. it's genuinely the best thing i've read lately. do it right now i'm very extremely serious. yes this is the bigolas dickolas tweet.
Kaijuu ni Natta Gay
