Heard that it's an adaptation of "The Greatest Robot on Earth" arc of Astro Boy. But I still don't get it, what does this mean? Is it a spin-off of Astro Boy or something? And does Astro Boy himself appear in Pluto?

It is kind of a spin off using some of the characters from Astro Boy plus some original characters, and yes, Astro Boy does appear

Astro boy is called ‘Atom’ in Japan. He is literally the the kid in the cover.

You don't have to have watched Astro Boy to enjoy Pluto. It's basically a darker retelling of one of the arcs from Astro Boy, but with the focus now put on the other characters, and Astro not playing as big of a part at all. As long as you understand that it's related in some way then you can enjoy this fine, I think, though it's better imo if you know a bit about the original too. This was one of the big arcs for the original Astro Boy manga, so it's a memorable part of the series.

Astro boy's japanese name is atom
It can still be enjoyed even with no prior knowledge about astro boy

I never watched astro boy, so will I have any problem watching this?

I think you can watch this fine, just basic knowledge that Astro Boy exists is enough.

No Astro Boy is for kids and it's also really old most people that have read this have never watched Astro Boy it's completely fine to go in blind. Not saying Astro Boy isn't worth watching/reading but it's six decades old at this point depends on your taste.

I have to agree. I had an easier time getting into the version that's only 2 decades old.

If anything is probably better if you didnt read astro boy before this, its a loving homage but its extending a far shorter story arc and developing characters that only appear for a few panels into absurdly fleshed out characters and by extension its alot more suspenseful if you don't know the basic plot outline. Just knowing what astro boy is will more than suffice as the story does such an excellent job building up the world from merely the first chapter.

The Greatest Robot in the World arc is only about 180 pages in Tezuka's manga. You can find it in volume 3 in the Dark Horse edition of the Astro Boy manga (your local library may have it on a digital service).

I wouldn't expect anyone to read all of Astro Boy for Pluto (I haven't) but I would say at least that is worth a read. It's fun to see how Urasawa works from the source material, and will give you a better idea of what's happening when it gets confusing. You don't need to worry about spoilers, Pluto is told in such a different way, with some things being changed and enough additions, that even though I knew Tezuka's original story I was left wondering how things would go.

More Astro Boy might give you some more appreciation for the world the story takes place in. And some plot points that are not in the Greatest Robot in the World arc also come up in Pluto, there are also quite a few easter eggs, some of which I caught and others I did not.

If you're curious about the anime adaptations, I know the Greatest Robot in the World arc only makes up two episodes in the 1980s series, but some key parts of the manga got cut from that, and they are incorporated into Pluto. The 2003's version sounds way too different. I am unsure of the 1960s version, or if that part even made it into the dub.
