Idk about her character later, but I watched the first episode and got infuriated by her. She does some good things, like helping her hikikomori brother-turned-girl getting out of his house and do sports, and she might do other good things on later episodes, I'm sure.

However, I can't help but get infuriated by her. She is manipulative and a sociopath, I guess? She drugged her brother and forced him to live a girl lifestyle (e.g wearing female clothes) by throwing all his male clothes.

It might not be that deep but I just can't help but think so. Do you feel the same way?

Yes, she is basically Makima, who also used mentally weak and miserable male and manipulated him into being her toy.

The end goal might be that she wants him to improve overall best for him, but whole series is based literally on her doing something so incredibly messed up, that it's really off putting and kinds diminishes supposed wholesomeness.

If everything depended on Mahiro, he'd refuse to do anything at all. She not only did some good things, the transformation forced the MC to move out of his comfort zone. Mihari threw Mahiro's clothes because she's now a girl. It may be questionable what she does but she didn't do anything wrong. The drug's effect is also not permanent.
If you have time to think of a beautiful end, then live beautifully until the end.

Yeah she sucks but whatever she created a cute anime girl potion. She is also trying to improve her brothers life. Which is kinda touching

It’s a conflicting plot for sure. I had moments of thought throughout the episode which went from “hah, this is funny, cute…” to “hmm, this is kinda painful to see the oniichan potentially suffering this much”.

Best thing to do is probably not take it too seriously and just enjoy the comedic perspective of it as best you can. I doubt anyone would find a real life story of someone being forced to genderswap by their sibling through an experiment very amusing. Love that anime pushes boundaries that real society wouldn’t dare to. Since it’s fictional there is no actual harm.

I mean, if her manipulation ends up helping Mahiro, I don't see a problem with it. Besides this is a comedy based series, her characteristics are nothing more than comic relief.

Did he return to male in the manga?

No, if anything it is heavily implied Mahiro wants to remain a girl forever.

I also like that anime is willing to push not just the physical laws of the world, but the social norms too.

I would guess some of the anime-watching audience is so starved for companionship that they'll accept even a Nagatoro-san or Uzaki-chan.
