Something I've always wondered is why or how did Shigechi get spikes on his head? Was it a medical condition, etc.? Or is he like Bart Simpson, looks like he has spikes, but is his hair?

Hirohiko Araki adds a lot of strange characteristics which are entirely for visual purpose. Example: Koichi Hirose's height. In reality, he's as tall as Josuke. However, Araki draws him short to show that he has really low self confidence and feels everyone is towering over him. I believe Shigechi is a similar case, so basically just don't think too hard about it.

Yeah I guess it’s exactly that. I guess also that he’s generally off putting and you don’t wanna come near him on first impression, but he’s a sweet enough kid at the end of the day. And so that’s very clearly represented in the visual aspect. A lot of JJBA designs are deliberate like that. You can discern every major characters personality or context from their design.

josuke is a lot taller than koichi but koichi is drawn a lot smaller than he actually is for maybe the reason you said
