It's not a fantasy world; Ragna Crimson is happening in the present world. In other words, America and the other countries of this world exist, but some countries, like America, are in ruins due to a war that happened in the past. And the cars exist because Crimson is able to store those things in another dimension with his magic.
The anime forgot to adapt an important part from the flashback of Crimson, which happened right after Ragna vs Ultimatia.
