There's always a game that you've missed over the years, or many. There is definitely loads that i have missed. So recommend me some lesser known games :)

Not really looking for JRPGs, but fire away with everything you've got :D

Does anyone else need a doctor to confirm that someone without a head is dead?

Anything really, as long as it isn't some really obscure thing that you can barely get or can't emulate on a PC :D

Tekken, lesser known D: Tekken is probably my 2nd or 3rd favorite fighting game series ^^

Shadow of the Colossus for an epic action/adventure game for PS2, this one is kind of storied though so maybe it's not lesser known.

Jet Set Radio a platform/action game for dreamcast, tons of fun, many memories hanging out with my buds grinding rails and spraying tags

Cave Story is another platformer but one with an actually interesting and compelling story along with a great soundtrack and classic metroidvania type gameplay.

Riviera: The Promised Land is an RPG for Gameboy Advanced but the gameplay is SO different from any other RPG I have ever played. It's not at all like a traditional JRPG more an RPG with elements of strategy and even a bit of dating-sim stuff going on. I thoroughly enjoyed this game and almost never hear anyone mention it, it was such a breathe of fresh air for me regarding the RPG genre that I'd encourage anyone to give it a try.

I used a GBA emulator when I played it, I'm curious does the PSP version include anything new or mostly just updated graphics and audio?

The PSP version has voice acting and some extra scenes. The graphics are pretty much the same though.

Gunstar Heroes (Sega Gen)
Demon's Crest (SNES) (one of Capcoms best games they've ever released as far as I see it)
Secret of Evermore (SNES)
Silent Bomber (PS1) (a really fine action game ;) )
Alundra (PS1) (from the makers of Landstalker and probably better than any Zelda game)
Harvest Moon 64 or Back to Nature (N64 and PS1)
Lone Survivor (PC) (a Silent Hill-esque game)

IF you like Zelda, then Alundra is pretty much a must play game.

That sounds pretty cool though, the voice acting was really limited in the GBA version as I recall so I may end up checking that out at some point.

Divine Divinity (PC) It has a good story and open world

Blood Bowl: Chaos Edition (PC) just a fun sports themed game, also a good game when you feel like smashing heads and killing people. It happens often.

Cotton Fantastic Night Dreams (TCD/PS1), Panarama Cotton (Mega Drive), Metal Max Returns (SNES, there is an English Translation for it) and Cotton 100% (SFC/SNES and PS1), the Cotton games are shmups so no Japanese is needed to play.

I don't know if it really is lesser known, but most of my friends haven't played it, except for me that is.
On that note I'll recommend Planescape: Tornment. It's an old game, but you can get it for cheap on Good Old Games. The story of that game is one of the most interesting I've ever played.
