Maybe I'm mistaken, but I've noticed something.
In this picture:

I think "Yuito" should be older than this. If we assumed that "Kohaku" and "Yuito" in the same age (I think she calls him "senpai") and if "Kohaku" got married, and then, her son or daughter will get married too, after all that "Hitomi" will be born. So, I think (In this picture) the age gap between "Hitomi" and "Yuito" will be (40-45) years at the moment she's born. I believe the picture doesn't match this truth, did I miss something?

This isn't a literal representation, is a way of showing that the reason he made this book (better, the reason They made this book) was because it was a gift for her.

I thought about that at first, but how did they meet? OR i.e. how did it (the gift) reach her? If that happened by Kohaku (or by her mother or anyone else), won't make it distinctive because giving something to someone will make it special only in case that "someone" receives that gift directly from the person. I think that will make the gift more valuable and memorable. This is why I believed he met her and gave her that drawing book and he should be older than that.

I noticed this scene and IMO the makers of this show meant it to be a metaphor for Him writing the book for Her. You see a young him working on something while Hitomi is reading the book(ep13 about 20:30), which I assume is the book. I imagine he gave it to Kohaku who gave it to Hitomi/Hitomi's mom at some point. Hitomi said she thought it was the only book she could see in color because her mother read it to her a lot.
They could use it as an "easter egg" or clue to a sequel where Yuito does end up in that time to present the book to a young Hitomi.
