Let me first start by saying that In that seen where Asakura reappears, Nagato had just altered the space time continuum. (when she was waving her hands in the air) so in that scene they went from being in the "normal time" to the "altered time" that is why that crazy bitch reappears.

Now regarding the stab wound incident, the Kyon he saw standing over him when he got stabbed fixed this entire ordeal by shooting Nagato with that needle gun reverting everything back to normal. So it was as if the whole stabbing incident didn't happen and was replaced by him falling down the stairs.

If you're still confused message me back what you still dont get and I'll try explain it some more. That's the thing about time travel plots, you got to pay attention to EVERYTHING lol
