Something that addresses sexism and abuse and homophobia and rape? Where those things aren't considered OK.

Boku dake ga Inai Machi has a child in it who is abused by her mother, and it isn't treated as okay. A main plot point is people working to take preventative measures.

Bokurano addresses some of this and although it doesn't treat it as 'okay'


Romeo no Aoi Sora also addresses abuse a bit, but it's something characters have to endure for an extended time.

IIRC, Michiko to Hatchin and Nabari no Ou address abuse a bit but it's been a long time for me with both those series.

+1 for @Kalendan 's recommendations.

Bokura no Hentai covers everything.

Kaze to Ki no Uta. I've only watched the movie (which covers a short part of the manga) and read the first volume, but it seems to cover every points, except the sexism.
In similar fashion, Thomas no Shinzou cover the abuse part pretty well.

And staying with the Year 24 Group's tragedies, Oniisama e… is a must, concerning abusive relationship.

Ima, Soko ni Iru Boku

It addresses so many sensitive topics like rape, child slaves, genocide, pregnancy as result of a rape, abuse and neglect.

Bitter Virgin (rape, child abuse, teen pregnancy), Deep Love [All three parts] (rape, pregnancy, prostitution), Mawaru Penguindrum (rape, child abuse)
Not sure about homophobia though...

Abuse wise, I've recently been watching Your Lie In April. One of the anime's main focuses is the protagonist's recovery from the effects of his mothers abuse. I haven't seen all of it, but so far, it's super good.
