Why is it "Oshi no Ko", and not just Oshi no Ko? Is there a narrative reason? Is it to push it to the front in Alphabetical order? Any theories? Did the author say why? Is it spoilers?

Yes. Iirc, Akasaka-sensei said that the title is stylized as "Oshi no Ko" due to narrative reasons. He didn't give any clues because it will spoil the story. I can't remember where I read it, though.

The Japanese title is 【推しの子】, or 【Oshi no Ko】.

I believe it's just for stylistic reasons. It's a pretty short title, so they may have wanted to give it a little more flair to make it stand out. Wiktionary has this to say about those brackets:
Punctuation mark
【 】

  1. Encloses a heading, such as a dictionary headword.

So maybe it's just to emphasise that it's a title, and not part of a sentence or something? ~ ehhh.

English doesn't have an exact analogue for those punctuation marks, and I don't know why MAL decided to use "quotes" instead of [brackets] or just dropping them altogether. (Stuff that we would put in quotations marks usually look 「like this」in Japanese instead.)

[EDIT] Oh, right, I vaguely remember what the previous poster said. Combined with the above, if I had to guess it might mean that it's a title or chapter of an in-universe work of fiction or something. Nothing that's been covered in the manga so far, though.

appendix of chapter 15:

As you might know, the title of the manga is 推しの子 (oshi no ko). Let’s break this title down a bit.

Literally, 推し (oshi) means ‘to push’. It can also refer to the act of supporting someone; this is similar to when you “stan” a celebrity. This word is used a lot by idol fans to refer to their favorites.

子 (ko) has several meanings. One meaning is to refer to a person, usually a young woman. So putting them together, you get 推しの子 (oshi no ko), meaning the girl that you support. In this case, it refers to Ai—the person that Aqua and Ruby supported in their previous lives. However, it also has another, less obvious meaning:

Oshi can be used on its own to refer to your favorite person among a group (in this case idols)

Another meaning of ko is the child (of someone)

So putting them together, you get “推し”の子 (“oshi” no ko), meaning the child of the idol that you support. Here, it refers to Ai’s children, Aqua and Ruby.

Tldr: The quotes most likely are there to annunciate the dual meaning of oshi no ko; but this is just speculation. Maybe it’s something plot related. Hope this helped.

I misunderstood your question. I thought you're asking about the quotations, but you're asking the meaning behind the title itself. The title's meaning is already explained above me. As for the double quotations, My answer is still the same.
