like what exactly causes the mc to suffer? is it because the girl doesn't like him back? she's not with him? she likes someone else? elaborate a bit more...

My favorite anime where the MC male is a bit self-tortuous is Devils Line. It is a vampire anime, and the romance is a lil spicy and more adult-ish, but I liked all of the characters. Especially Anzai, the male MC.
