I'm looking for manga that portray realistic relationship between adults or deal with issues such as loneliness, mental disabilities, or gender identity. It should have little to zero comedy.

edit: slice of life and drama only

I should have added slice of life and drama only when i posted the thread. Not quite what I'm looking for though but thanks.

hmm, well most Osamu Tezuka stuff comes to mind, most of his drama stuff at least.
For some shorter ones, try:
MW (mental disorders, gender identity)
Ode to Kirihito (treatment of people with physical ailments/disabilities)

For some longer stuff (not Tezuka)
Please Save my Earth (gender identity questioning caused by a past life, one character spends 9 years in solitude by himself and goes crazy), even though the primary cast is high schoolers, it doesn't feel like high schoolers because they don't spend most of their time worrying about school. A few comedic moments pop up earlier in the series, but they tend to not appear as much as the series goes on. Sci-fi/drama/romance (not as big as the other two genres). All past life stuff occurs with people older than college students.

@adamantine: I'm on volume 2. I just haven't got around to update the manga list, I'm so lazy. That being said, I really like I.S.
@tehnominator: I'm currently reading that too, lol... I'm not sure if that would count. Can you give me an example of one?

I won't write any synopsis for any of them. Note that they are ordered in terms of relative importance and ease to find (except the last two, I've included them because they are really good)

I second this recommendation, and also 'Mars' by the same author. Not enough readers for these excellent mangas.

Third Eternal Sabbath, and second Mars.

Unless you disliked Pietà, you should absolutely read Double House by the same author.

River's Edge is less about adults, but definitely about loneliness and somewhat about gender.

And for a bit more of a reach, Yokohama Kaidashi Kikou is slice of life with the connections and separations between people as an important theme, definitely worth a look.
