I think she was pregnant. They animated how Sekai realize it when she was vomitting. So I don't think She was lying. Kotonoha said she wasn't but she has gone mad as when she was talking to her phone.

But maybe they did it on purpose, so the viewer can decide what he wants to believe.

Although makoto is so stupid that he may be missing like 3-5 chromosomes so who knows if he can reproduce at all
