I like how even the doctor's trying to hold back his laughter when seeing the state our protagonist is in. But it seems all the nurse did was get the protagonist off. It's very strange that the phallus on his forehead ejaculated though. It would mean semen somehow traveled all the way from his scrotum to up there (which would be impossible unless he's got another vas deferens that connects the two parts).
