What he's going to do to the Leaf, can't be determined. I know he has a problem with the elders, and if he ever does do anything to the village, other ninja could obviously be caught up in it. To my knowledge though, and I'm trying to recall anything...but, I can't, so I'm not sure about civillians or whatever.

Has he killed innocents up until now? No. None that I recall, anyway. So most likely not.

Would he? Based on his downward sprial, it could happen. The way things have been going, honestly, I wouldn't be shocked.
