Uh, he's crazy? Plenty of people get obsessed with wanting to be different, get addicted to cosmetic surgery, and all kinds of similar shit. I don't think you're supposed to understand him.

I don't think he's that unrealistic... he's only mentally sick. He has probably BDD or something like that + he is a pedophile. Not that unrealistic after all.

I think he was someone whose obsession over the Phantomhives drove him mad. After meeting Vincent, he was not only obsessed with him but also with the idea of being an evil nobleman. He wanted to be more like him, so that Vincent would acknowledge him. And I also think he was attracted to Vincent and Ciel. After Vincent dies and word got out that one of his sons were alive, he directed his obsession onto Ciel. This lead him to get those surgeries. Since we all know how sick & twisted he is, he manipulated the Circus troupe to abduct those children.

In the end, he was self-loathing, perverted and sadistic scum bag murderer, just as Ciel describes him to be. More or less...
