Differences - The other one has dense MC, while the other one doesn't. - The other one have romantic moments with the boys the "heroine" is supposed to end up, while the other one doesn't. - The other one involves licking feet, the other one isn't. 

- Both feature female leads - Both main characters were reincarnated into worlds they already knew the future of beforehand - Both leads are extremely powerful (Lumiella in the more traditional sense, while Mia is the princess of a very influential kingdom) - Both mains have the ultimate goal of changing how their destinies play out using the knowledge from their past lives 

-Both protagonists are heavily discriminated against by having cursed origins. -Both protagonists are very diligent with training and getting strong. -They both enroll into magic school but are hilariously overpowered. -Similar nonchalant attitudes. 

They're the exact same anime but with the gender of the main characters reversed. Everything from the plot to the side characters is a direct copy. > Both of them are about the MC getting reincarnated into a game as a villain/side character. > Both of MCs are ultra OP. > Both of them have a female character who is surrounded by a male harem. Ton of other similarities. Too long to list out. Personally, Otome Game was more satisfying to watch. 

They both have a skilled heroine that is more focused on her goals than romance. While the plot is a looot more shallow in Akuyaku Reijou Level 99, while in Kusuriya you get properly emotionally invested, they both give off a similar vibe in a sense; so I feel that someone who enjoyed Akuyaku Reijou Level 99 will definitely enjoy Kusuriya no Hitorigoto even more. 

In both the FL got reincarnated into an Isekai after dying and maxed to level 99! the shows aren't really similar though. 

Both involve OP MCs who are just trying to live a normal school life but their cheat level powers draw unwanted attention.
