first let me start of by saying that I was rooting Inaba since the very beginning of series even before taichi and Iori started dating but what exactly did taichi x Iori relationship mean and what led them to ultimately break up?
Was it that that Inaba imprinted the idea of there relationship on them and or pressured them into it? Was it that she fulfilled his desire to be the savior of someone who really needed him, seeing as she was emotionally the weakest? Was it that they both realized that he just fell in love with his perception of whom he thought she was and didn't truly understand her? Could it be that she was just venerable when when asked her out? or all of the above?

At first I was also puzzled by this, but after reading some comments and part of the visual novel I understand now. To answer your question simply: Taichi was in love with the ideal image of Iori instead of the real her. Iori knew this and concluded that she didn't deserve his love, or rather didn't love that Taichi anymore. There was just too much that damaged their relationship.

Taichi and Inaba however, understood more and more how much they needed each other. Inaba is the one who made Taichi understand how much of a "selfless freak" he was, so Inaba is needed by Taichi to keep him in line. In turn Taichi, being the white knight he is never felt more needed by anyone than Inaba, which evoked the desire to protect her.

I recommend reading the novels, or at least Michi Random (the 4th one) in which I find this is much better explained.

I think it's because they never did have a chance to expand their relationship more.

The confession was interrupted with heartseed taking over Iori and the whole bridge incident. There had been a lot of phenomenons that would distract them and focus on other things (hence why Iori told them to hold off till it was all over).

They themselves never jumped at the chance to try anything too. Inaba had more time with Taichi from all that I seen and was more assertive to trying to win him over throughout the series (mutual masturbation like seriously). Plus when they did hook up things got calmer and rounded off to a happy ending.
Taichi didn't seem to care much either after the confession, he never did try to get closer nor help her closely especially in Michi Random (It felt not as strong as with what he done for Yui in the main series). Even Iori was pretty distant too.

Maybe I forget things since I did watch this a month ago, but the whole thing seemed doomed from the start and its rather tragic, I honestly wished they had a bit more development but now im ok with them not being together.
Help I have fallen into the Villainess genre and I refuse to get out

I never really liked Iori but after watching ep14-16, I finally understood her. The ending was just right for everyone.

But Iori practically forced that out of Inaba.

I'd have decked Iori for fucking with me I was Inaba, but I guess that's not how high school girls roll.
