Ok at the end of the Club URL put a "]" and remove the "[/url]" and put it at the end~

Here is example:

[url=http://myanimelist.net/clubs.php?cid=69338][IMG]http://i62.tinypic.com/35jlemg.jpg[/IMG][/url]

And if the Banner doesnt show up then try uploading the Banner again~
