Regarding about the disease in Tokidoki

It's called koku haku which is confession in Japanese. So why is it a disease that weakens the heart? I know that this a manga and perhaps it's just a random name but I still want to know why. Perhaps it's to be ironic??

Perhaps it has something to do with the kanji (lol). It kinda stuck out for me too so I'm guessing it's some kind of tl thing. Or the author just chose it to be blunt that it's no real.

I'd like to know about it if there's a disease that can kill you exactly after a certain number of heartbeats lol. It's obviously not real.
