Friendly-fire Anime

Ok, so for starters, while I have already made a topic (http://myanimelist.net/forum/?topicid=1391885) that involved friendly fire, it didn't have it in the title, so might not been having grabbing as much attention as it could have, and was centered much more around romance and a love triangle.

What I'm looking for (in order of priority, from highest to lowest) is:

Requirements:
1. An anime that involves friendly fire (not just friendly fire between random background characters, but friendly fire between two characters that both get a decent amount of screen time)

Ideals:
2. This friendly fire will kill one of the characters, not just injure them. (If he technically dies but comes back later [NOT STRAIGHT AWAY], I'm cool with that)
3. I would like a female MC to friendly fire a male MC
4. I would like that female MC to be in love with the male MC (whether it's one sided or not doesn't bother me, if she has feelings for him, that's all that matters)
5 (Although, if this is truly priority based I would have ranked it higher). It actually has to be a sad ending. I would like to see the female MC be mortified by what she's done.

Exceptions (I've already seen this, which have similar themes):
Maji de Watashi ni Koi Shinasai!
Date A Live
Sola

Any suggestions are appreciated. A rough indication of how much this fulfills my expectations would be apprecciated as well.

P.S. If there are a huge amount of spelling mistakes, I recently switched from Chrome (because it was playing up) to Firefox and haven't got round to turning on the spell-checker yet.

Have you heard of or tried Btooom! ?

The only downside is that the anime only has one season, so you have to actually read the manga to find out what happens from there.

Oh, and what about Mirai Nikki?
