As far as graphic violence in Japanese graphic novels go, these two are in the top-tier. Ichi has a modern setting and far more sexual content/rape, but Shigurui's ALMOST as fun to read as Ichi when it comes to OTT violence; body parts flying everything as both fists and swords kill everything. You could describe Shigurui as Ichi's samurai equivalent since the main draw of both is seeing people get brutally murdered and, strangely, enjoying it.

Main characters who are seemingly nice, shy and ordinary people, but in reality they're deranged sadistic killers out on a quest for revenge against those who tortured them in school.

Crude and rough drama that does not mind becoming violent or explicit, but is using said explicity not mainly for shock value, but as a tool to show human psyche. Follows cast of various characters which some of them (most) might have a screw loose, including the protagonist. Both manga are also beautifully drawn, and as they are made by the same author they also have similar artstyle and feel.

both protagonist are super strong martial artists and they live in a work full of sorrow and pain.. the underworld, where yakuza and other clans find their place. Both of the character show no mercy when they fight and even kill their opponent.

In both manga, the main characters are depraved criminals, and both manga focus on yakuza. Both manga have a lot of gore and nudity, as well as other taboo and mature themes. Both manga create a similar mood.

Both manga follow a depraved and sadistic hitman who kills his targets in brutal ways. Both manga have a lot of action and violent scenes, giving off a very similar vibe.

Abnormal-kei is a bit softer, but these two manga compete in the "sickest manga in the world" competition. Abnormal-kei is funny though, and more of a comedy.

If you, say, don't get nightmares from reading Ichi the killer, or you read it to get those nightmares, I'd say Usamaru Furuyas Garden might be straight up your street. Garden also gives out a certain indie vibe to it that might excuse the excessive indulgence of sex and violence.

Extra violent Yakuza flick with a one-army-man protagonist

Both main plots are about hitmen and their lives. Mc's have psychological issues.

MC strays from the normal path, becoming murderers, somehow with another personality. The authors visually don't shun sexually explicit scenes of rape and the likes. *As you could expect, these are both Seinen. (*As for Koroshiya 1: read the prequel as well, then the picture becomes one)

From the hand of Hideo Yamamoto another story full of Yakuzas and violence, another psychological manga with twisted characters. Máster Piece.

Oldboy and Ichi the Killer are similar in the sense that the majority of the plot revolves around the main character having their actions manipulated by somebody else. If you enjoyed the meticulous mindf*ckery from one of these series, I'm sure that you will find entertainment in the other. Ichi the Killer is undoubtedly more violent and action-packed, whereas Oldboy is more brooding and focuses on the psychology of the characters. Both titles have also been adapted into successful and well-known live-action feature films.

Both the protagonists are sadistic and perverted murder loving freaks while the MC of Dead Tube is superior in Intelligence levels and the MC of Koroshiya 1 is superior in Strength levels. The reader will get the outlook of gore published in the 90s through Koroshiya 1 and gore of the modern era through Dead Tube. Overall both of them are a must read if you're like me: a fan of seinen gore manga.

While Ichi the killer is running for the "sickest manga in the universe"-award, freesia is a bit more restrained, well not restrained, but moderate in the sense that it never tries to be disgusting for the sake of being disgusting, it's a bit more focused on the storytelling. That doesn't mean that Freesia deserves a spot on the list of "sick mangas you should read before you turn sane and don't enjoy this shit no more". Freesia has a bit more humour as well.

Both series are bloody violent and those looking for gore shouldn't look any further than these two.

Though Ichi's plot is a milk for babies comparing to MPD's crazy twists, the whole atmosphere of insanity is pretty the same. Madman killers represented by every second person in MPD and Ichi alone in Ichi, but bloodshed and gore are almost at the same level. So, if you enjoy mad murderers do their work check these series.

In both manga main characters are attacking Japanese mafia(Jakuza) and both of them have an enormous body count. But those are the only similarity there is. Main characters have different reasons for doing what they do. Bakune Young is much more comedy like so art stiles are completely different.

Both of these titles feature copious amounts of blood being splattered and countless lives lost.

Both follow a small organisation of men who interject themselves, in one way or another, with corrupt dealings among gangsters and the like.

Bloodshed, gore and almost hentai scenes bring these series together. Though plots are completely different, the whole atmosphere of madness and non-stop violence is pretty the same.

A similar psychological mangas, because of the plot and violent. In Ichi everything is about mafia and stuff, whili in Manhole it's about deadly virus.

Manga where the protagonist has conflicting appearance and actions. One has a protagonist that looks absurdly scary to others but in fact has a gentle heart, while the other one might appear as a crybaby but he is unleashing absurd carnage, both confusing the hell out of the rest of the cast.

So many things in common: agonizing torture, brutal combat, seedy underworld, genital mutilation! Even down to the same tricked out suit with embedded blades. Ichi the killer is way more unique and disturbing but The Scum makes for a great follow-up read.

What is perhaps interesting is that Ichi The Killer can pass as a story subset for Black Lagoon given its focus on yakuza, illegal dealings, and action.

They're both incredibly violent involve lots of adult situations and lots of morally wrong actions.

The protagonists in these two publications are highly skilled killers that often cry as a consequence of their actions. Mature themes are plentiful.
