To me it's between Another and Corpse Party. I couldn't stop cringing in some scenes of these shows and sometimes took a break from one episode because im scared of the next one

Mnemosyne, I think so far is the most gruesome anime I've seen. But the scenes of lesbian sex were incredible.

For me it has got to be Higurashi. Masterpiece of horror anime. Really knows how to build the ambience and the story arcs are scary af, some more than others tho (Eye Opening arc was some wild shit).

Higurashi or Ellen Lied for sure. People like to call those shows “edgy”, but it’s the haunting atmosphere and perfect pacing that really make them stand out from the rest. Although, I would say the best horror anime is Perfect Blue.

Genocyber, Mnemosyne, MD Geist and Wicked City. Nothing else comes close.

Probably Genocyber or Violence Jack, but they fucking sucked though. Also worth mention is Blood C because of that ending.

There is also Gyo: Tokyo Fish Attack, but that movie is a legit 1/10 and was just stupid and gross more so than gruesome.

As far as good gruesome anime goes, Elfen Lied, Higurashi, and Umineko are all way better.

Maybe Pupa? It's an interesting one for sure.

I'm not really bothered by gore though so I tend to watch a lot. In terms of sheer violence, Elfen Lied, Straight Jacket, and Higurashi I guess though Muv-luv Alternative did have it's moments. There were also parts of Shin Sekai Yori, like what happens to squealer at the end and the top warrior priest in the later half of the series.

Yosuga no Sora is probably the most "directly" sexual of everything I've watched, so it's probably got the most "squick" value for some people -- particularly its infamous inclusion of incest. However, while it's explicitly sexual, it's not visually gruesome; Cross Ange and (particularly) Brynhildr in the Darkness are probably more gruesome. Probably also Black Bullet.
Avatar character is Gabriel from Gabriel DropOut.

Elfen Lied and Pupa come to mind that I've completed, both are pretty shit tier. Gantz was at times from what I remember and of course the infamous Griffith episode in 97 Berserk.

Shigurui was so fucked up I needed to take breaks during and between the episodes, and this was the first time it happened to me with anime. It has really disturbing scenes that aren't just about gore and it relies on heavy atmosphere too so it works better for me than just a horror anime. But when it comes to gore alone, no matter how shitty Corpse Party is, it doesn't change the fact I've yet to find an anime with more gore than it and I've seen most of what people mentioned here.

Genocyber is probably the most violent and gritty show, but I saw it pretty late in my anime fandom, so it didn’t really shock me. Blood -C is one of the very early anime’s I’ve seen, like when I was still new and watching random stuff that looked interesting. It was probably the only anime to ever trick me into believing it was gonna be a slice of life with some supernatural fights, but I was very wrong. That shit traumatized me back then. I went in completely blind. Other anime’s like madoka magica or school live never had that punch that blood -c had on me because I went in expecting shit to get dark. They’re great shows, but I knew what I was getting into because I’ve seen the genres they had, and other people have told me they were dark. Blood -c still sticks with me on how it made me feel back then. If I were to watch it today for the first time, it probably wouldn’t phase me though, but back then it really did.

I was gonna say this, as it was the first thing to pop into my mind (One of my favorite shows) but reflecting on the show there really aren't that many gruesome scenes in the show. The most gruesome scene was probably:

The rest was actually quite tame compared to what people said happened in the show.

I remember thinking that "Akira" had a lot of violence when I first watched it back in the mid-90s, but after watching the 10th episode of "Made in Abyss," I can definitely say that I was far more bothered by that than anything I saw in Akira.

I agree Daughters of Mnemosyne was brutal in its graphic content. However the "Violence Jack" series really takes the cake blood and gore in an anime I ever seen.

Bingo! Shock value schlock really is a dime a dozen. Hey, remember that Owari no Seraph or whatever where they kill a gaggle of kids in the first episode just for the MC to give a shit? Oh, neither do I, but I do remember I broke out laughing.

That said, VNs really do brutal stuff more justice than manga or anime. I'll never forget the cannibal arc in Grisaia, but the anime really didn't do it justice. (Like it didn't do anything justice)
