You can read on Black Lagoon Wiki "In a series of drawings made by the mangaka hiro reí in comicket 83 is reveal that both twins are actually females in the drawing can be read the next paragraph "The twins who showed up in season 3. They're real twins of German descent. In the story, they have differing genders, but actually both of the actresses are girls." |

| The manga doesn't give any added context, the anime is very close to the manga overall. I originally thought they did some underground body "modifications" when I saw the anime but I think Rock is just really upset and sad about the whole situation. Revy is basically unfazed and is more mad that Rock is upset than surprised or disgusted. |
