Crunchyroll now has the series less one missing episode. (I haven't yet figured out which number the missing episode is, but the last episode on Crunchyroll appears to be the final one.)

Episode 15 from the original order is removed. So the order is:
Ep 16 of MAL discussion = Ep 15 of crunchyroll order
Ep 17 of MAL = Ep 16 of crunchyroll
...
Ep 43 of MAL = Ep 42 of crunchyroll
