Nana, the 40ish adapted chapters don't skip anything, even the sexy scenes.

Honey and Clover too. Story even ended in the first chapters of final volume (volume 8) to give the anime time to catch up with the ending. Rest of the volume are side stories

As bad as Berserk 2016 and 17 were, they were very close to the manga. Minus all the detail though...

They are adapted panel by panel, and even adding more stuff to make them more fluent

In both cases, Anime > Manga

I was just speaking in terms of the scenes it DID adapt. I'm well aware of what they completely ignored. T_T

ah ok.
At least they could have mentioned the ignored fight in the first episode or shown a still picture. Guts is presented as being just tired from his travels, when in fact he got his ass kicked by Rosinne.

Hellsing: Ultimate is the only one that comes to mind that is completely, 100% faithful, even down to some jarring humor scenes that don't really work in anime form.

After certain point (when they decided that to continue with the Black Organisation arcs) and if we ignore the anime original episodes Detective Conan is pretty much page by page adaption.

The mangaka even creates certain characters with the clear idea about the voice actor who will play the role.
For example the character named Wataru Takagi is voiced by the seiyuu Wataru Takagi or Minami Takayama (the voice of Conan) also appears as herself (in the manga, later in the anime).
