The faces on the door shows where they are going (you can confirm it if you rewatch the episodes), so it looks like Onna was sent to reincarnation... but also, she failed the morality test and selfishly wanted to kill someone to go back to life even after she had suicide, and also cowardly asked Decim to press the button for her - I thought one of the reasons Decim was crying was because she failed the test... so, why do you think she was reincarnated? It doesn't matter if the judge do wrong (as showed after the first judgment) but doens't look right if he does that on purpose because of personal feelings... was it what Oculus tried to say? Or do you read everything that happened in some other way?

The arbiters seem to judge people however they want, that is why the system is faulty.
I'd assume Decim just wanted some kind of confirmation on sending her to reincarnation and he took her not pressing the button herself as that. He would have never send her to the void, no matter what she did.

But if we put it into perspective too, altho it would contradict all the other judgments, as it's not an western point of view of heaven and hell, but eastern of reincarnation and nothingness, the whole objective of existence would be ending the circle of reincarnation and achieve the nothingness, at least if we look at it as buddhists... so reincarnation can be a punishment :P
Eitherway, they really shouldn't have show us the face on the door, and should let us only imagine what happened.

But the void has clearly been established as the punishment of the two. In buddhism you go to Nirvana and are at peace, in the void you will suffer eternally.
Not showing which way she goes would have actually been pretty interesting (it would still be obvious though).
