Right click on the madVR tray icon,
Edit madVR Settings...
Under Scaling algorithms
image upscaling --> set it to Lanczos (3 taps, untick activate both anti-ringing filter and scale in linear light options)
image downscaling --> set it to Bicubic (3 taps, untick activate both anti-ringing filter and scale in linear light options)

Assuming you are using MPC, press Ctrl+J and an overlay will pop up. Both dropped frames and delayed frames should be 0 after playing the vid for a while.

the 480p video you have might be corrupted, try re-downloading it

Or madVR is upscaling the vid to fit the screen.

Sure.

If you don't want MPC to upscale videos, right click on MPC video screen while playing a video file and a drop down menu will appear. Go to Video Frame and select Normal Size.
