Seeing the names Tomoyo and Tomoya was a bit confusing inside Clannad to begin with.
The names of Sugu and Suguha in SAO was also kind weird too.

Names shared across anime, I don't find that weird.

Have I ever tried to feel awkward because some character has the same name as another character? No, names are not unique and over time I've become used to characters and even people occasionally sharing a name. I may think about similarities, differences, or possibly nicknames to distinguish between the characters but none of that causes me to feel awkward.

I tend to use full names when this happens, or reference the anime/manga they were from. So no, not really.
I like pie.

If not subs I would have really been confused which Shiki they were talking about there :D (is this a spoiler?)

Edit: Same goes for Tsukihime VN

What happens if their full names are the same?

Well, I just found weird that Dokuro-chan's main male character's name was Sakura. Always thought it was a girl's name.

Name Sakura. Well not really awkward but...

I think of cute and adorable Cardcaptor Sakura, and then bitchy Sakura from Naruto pops into my mind xD

Anytime there's a girl in an anime named Ichigo, I am silently making fun of Ichigo Kurosaki (Bleach) in my head.

I remember Shindou Hikaru and Shidou Hikaru from Hikaru no Go and Magic Knight Rayearth. Then there was Shinji Ikari from Evangelion and Shiji Hikari from Sei Juushi Bismarck. I found it slightly funny, but that's about it.

It is a bit annoying having two favorite characters named Haruhi, but it's not too bad.

Shuffle had the male MC and one of the harem girls both go by Rin. I guess they were referred to by different honorifics though.

Nope, never felt weird.... Guess I could always just tell (If it was the same anime.) or had the other anime entirely out of my mind.
Hm, looks like I can't get my signature to work anymore.

They're historical characters. There are also a common set of same names appearing in Sengoku and Romance of Three Kingdoms themed works.

Maybe Ruka from Vampire Knight because it means 'hand' in my mother language. Though it's not what the OP is asking for.
Also Alois from Kuroshitsuji because it was a name of my grandfather.
Can't think of anything else really.
The only thing you can rely on is that you can't rely on anything.

There are many Japanese names that are commonly used. But for shows that use more American names, it can get weird. For example, there's Nina in Code Geass and Nina in Monster... they are completely different characters, but personally, I absolutely despise one of them, and really like the other. So when rewatching the shows, for one I'll just hate the very name Nina, but in another I might just feel sorry for all the crap that shows' Nina goes through whenever she's mentioned.

Tatsumi Oga from Beelzebub and Tatsumi from Akame Ga Kiru share the same name and nothing really strikes me awkward. It just strikes me as kind of something to marvel at. Both use borrowed powers from an outside source is something I can relate to the two.

The most awkward of ones I know in anime is probably Shiro from Deadman Wonderland and Shiro from Ao No Exorcist.
Shiro from (DW) is one heck of a violent chick on her other revert personality and Shiro is a young female.
Shiro Fujimoto on the other hand is just an exorcist who isn't gruesome to her extreme and is a OLD guy.

Some characters with names outside of anime that are in movies are Alice from Resident Evil and Alice from Alice in Wonderland
Alice from Alice in Wonderland is a proper lady who just explores a dreamlike world and is pretty innocent and is gonna marry.
Alice from Resident Evil can't marry because she'll be the only one who lives out of the two since this world his a gruesome world filled with hungry zombies. Alice here is not innocent but one badass mofo.
