Both set in the Victorian era and both the lead characters hold the title Earl. Rather dark in certain premises though Kuroshitstuji has more humour in it.
Let's not forget the super loyal and capable butlers in both the manga. Another note, both Cain and Ciel have 'scars' at their backs where they refused anyone to see it other than their respective butlers.
Victorian England setting, nobleman and butler / master and servant relationship between the main characters, mysterious crimes occurring in town, gothic atmosphere, fascinating plot twists - both share these elements although The Cain Saga retains a higher, much graphic degree of violence and grotesqueness.

If you liked Count Cain Saga (and it's sequel: God Child), I highly recomend you Kuroshitsuji (AKA, Black Butler). Both of them feature a victorian aristocrat and his loyal butler as main characters.
Both of them involve mystery and dark organizations.
Plus, both series (and main characters) share a very similar ending.
For all lovers of Victorian Era, I suggest you to read it!

Both are based on young men who take over as head of their blue-blood, aristocratic English household. In both, these men are largely without family except for a highly mysterious butler who is by them at all times and who seems to excel at almost everything. The bond between Master and Butler in both series are of highlight because of how deep and unique they are. It really wouldn't be a stretch to say that Yana Toboso was definitely inspired by the Cain Series when she came up with Kuroshitsuji. The similarities are striking - a teenage Earl who lives in Victorian England, who has an abusive past that he goes out of his way to hide from everybody, except his super loyal butler, who knows everything to know about his young master.
