The biggest waste of potential? Aside from the obvious ones (i.e. Bleach and Naruto), I'd have to say the multitude of harem fanservice romantic comedies that don't really have any point to them other than to sell merchandise.
Haha, literally hundreds. I think I'm more surprised when there's a 12-13 episode anime that isn't rushed. I'm more used to the length of video games, visual novels and western TV which can often reach up to 50 hours (equivalent to 125 episodes of anime), so it's very difficult for me to get attached to the characters in anime when they're gone so fast. Anime is expensive to produce, though, so there's not much that anime studios can do about it, sadly.
It depends. There's quite a few decent anime with many episodes (Touch, Hunter x Hunter, Legend of the Galactic Heroes, Diamond no Ace, FMA Brotherhood, Aikatsu, etc.) but then there's stuff that airs simply for the sake of airing, such as One Piece and Naruto. And then you have something like Space Brothers which was a good anime crippled and pulled into mediocrity by its large episode count. As long as it's paced well and respects the viewer's time then a high episode count can be a good thing as it gives more time for the setting and the characters to develop. 13 episode shows are too rushed for my liking.
