The first arc of Gacha Gacha (capsule) deals with multiple personalities similar to Change 123. In each story, the male lead is trusted with keeping this fact a secret from their classmates. This results in both our young friends being put through some tough circumstances for the girl they have fallen for.
Two very similar mangas, whom both share lots of elements; martial arts, ecchi, love,comedy, drama, masters, and a love interest for our hero who can beat him five kinds of silly. To sum it up; Change 123 is the more mature, adult version of Kenichi and by far superior.
Both deal with a guy who meets a girl with a mysterious background...

while Mahoraba is more on the lighter side to Change 123's non-stop action/violence, both still deal with plot around the ideas of having to handle multiple personalities

Action and love story about a weakling boy and a powerful girl with some serious mental problems. Although male character is a nice person, in both mangas all attention focus on female characters. In C123 Motoko has divided personality and in Mirai Nikki Yuno is superprotective and obsessed yandere who has no qualms about killing. Both has action and blood, the difference is that there's more ecchi and martial arts in C123 and Mirai Nikki is more violent and even creepy sometimes. But if you enjoy one, I definitely recommend the other.

The author of Change 123 has admitted that Getter Robo was one of the main influences on the series. While they're otherwise very different (Change 123 is about girls and martial artists, Getter Robo is about men and giant robots), they both feature main characters who can switch between 3 forms in battle to deal with different situations. Also, the three fathers in Change 123 are huge homages to the three main characters of Getter Robo.
Both have a mix of comedy, ecchi and martial arts. Although two different themes, the feeling the stories transmit are somewhat similar.
These both share the common element of a split personality, with the base personality being quiet/shy and the split personality being violent/crazy. Change is way ecchi, but they both deal with a kind hearted guy who is trying to help the girl out.
they are both ecchi (lots of breast), action packed, with some great drawing styles. Seikon no Qwaser does have a bit more serious tone usually, but if you like one you'll probably like the other
