Okay, this is how this works; fill out the "Membership ID Request Form" and post your ID request here via comment. When I get to your ID request I will take that info and make your ID card. Once I've finished it, I'll send it to your profile via comment or personal message, and then I will cross-out your request here.

Now a few spots were reserved for the officers and admins of the club. All officers and admins should have received a message from Arestyr, if you didn't respond I have not made you ID Card yet, please request your ID Card regular way now by posting here.

Member No: Please follow in numerical order. Since there were spots reserved for Officers/Admins, the next person to request his or her ID Card will be, "Member No. #0008" then the next person would be, "Member No. #0009" and then next would be, "Member No. #0010" and so on an so forth, 11, 12, 13, 15, etc....

Name: It can be your real name or the user name you use here.

Member Rank: Whatever your rank is in the club. If you’re an Officer, your rank would be 'Officer'. If you’re an Admin, then your rank would be 'Admin'.
If you're a regular member... :) FEEL FREE TO MAKE UP YOUR OWN RANKS! :D
Examples, if you feel slow and can't think of a cool rank:

~ Fangirl
~ Fanboy
~ Fuko's Friend

As long as it's not anything vulgar, i don't really mind what you request what I put on the card as your rank~

Template: Please limit the number of templates you choose to two only~ If you choose more than two, I shall get mad >D But, if you do want more than two, leave me a PM or comment on my profile and I'll try and get to your request as soon as possible.
