Apparently it is Konoe but they are not biologically related. So can someone please explain the whole situation to me.

Basically there is no little sister. Konoe is the biologically daughter of his father but it turns out that wasn't his real father. Instead they had trouble conceiving so they used a donor.Miyabi is his cousin and becomes his step-sister when her father marries his mother. He chooses Konoe in the end.
