If anyone stumbles across this, Nyarlathotep and Mahiro get married and she f*cks him to insanity, with things like "No", "Help", "I can't do this anymore" and "I'm dying" being heard from their room. I do not know if the insanity is permanent though, but it is supposed to be a happy ending, so it most probably isn't. You know what they say: can't spell "Nyarlathotep" without "thot".

What? a guy said on Reddit that they have sex at the end of the volume, in this part the mahiro is screaming it's because of the sex the two of them have and not something else.
