Kaoru is the younger twin brother of Hikaru Hitachiin, by three minutes. He and Hikaru are both in their first year at Ouran and are classmates of Haruhi Fujioka.

Initially, Kaoru and Hikaru felt Haruhi's presence in the Host Club was questionable. A main contributing factor to this was that she could tell both twins apart with ease. To them, it was one of the first times that anyone had ever stepped into their world. Before Haruhi and the Host Club, the twins thought of themselves as "our world" or outsiders as "everything outside our world" because they never had any personal relationships with anyone else.

In the anime, it seems Kaoru wears orange apparel when outside school grounds. Examples of this can be seen in episodes 7, 8, and 19. Kaoru and Haruhi are currently close friends.

As Haruhi points out, Kaoru is not as ridiculously hot headed (even though they look the same) as Hikaru. Kaoru is more thoughtful and considerate of the feelings of others. Otherwise, Kaoru is similar in many ways, and likes the same things as Hikaru, such as maple syrup, Italian and spicy foods.

Kaoru is also the twin that understands that it's not healthy for the twins to be so co-dependant and that they will eventually be apart - if they were to say fall in love and become part of a new family.

If both the twins desire something, and there is only one of that item, Kaoru usually steps back and allows his brother to go forward, refusing to compete with his brother either directly, or going behind his brothers back.

Currently Kaoru is undergoing a transformation in order to separate himself from his brother. Aspects of his personality such as the fact that he is the sneakier of the two twins. With the sports tournament arc he is choosing to legimately compete against his brother as part of a different team.

In the manga, both twins like Haruhi. Although Kaoru realised his feelings first, he also realised that his brother Hikaru also liked Haruhi and choose to step back and allow his brother to realize his feelings and move forward. As of volume 8 both Hunny and Mori know of Hikaru's crush on Haruhi, as well as suspecting Kaoru of holding similar feelings. Kaoru, in the most recent chapter, finally told Haruhi his feelings, also giving her a kiss on the cheek. He then stated he was not prepared to follow through though because he cared about his brother, Hikaru, more than he did her. It is also mentioned in the manga that he does not like Haruhi as much as his brother Hikaru does. As his brother states, when he saw Haruhi with Tamaki he was jealous, and when he saw Hikaru with Haruhi he was very angry with him. In which Hikaru replies with, "If I were in your position, I don't think I would have been hurt" (the position being seeing Kaoru kiss Haruhi on the cheek). His hair parts to the right(his right)
